Appointments made in advance of 24 hours or more:

A $50 deposit per person deposit fee is required. Directions to completing this deposit will be emailed directly to you to complete prior to appointment. This will go towards your total cost.

We understand life happens! If you need to cancel or reschedule your appointment, please email mobileiv@besowb.com or text (602)-633-5194, 3 hours in advance to let us know. Any cancellations within 3 hours of appointment time will not be subjected to a refund. Rescheduling will not result in a penalty.

Appointments made within 24 hours of desired appointment time:

A confirmation email or text will be sent after appointment booking. A $50 per person deposit fee is still required prior to RN arrival. This will go towards your total cost. RN will offer a 15 minute grace period after start of appointment time. Once the 15 minute grace period is up, it is under the discretion of your mobile IV nurse to wait longer. Deposits will not be refunded.

Consent:

Each participate receiving an IV will sign a consent form to authorize RN to perform IV insertion and infusion. Forms disclose consent agreement, potential risks and side effects, payments, and arbitration agreement. It is your responsibility to ensure the form is understood and read entirely. Feel free to reach out to our staff with any questions regarding these forms.

In addition, each participant will fill out a basic Demographic and Health Information Form to ensure the safety of the client. Please read all forms thoroughly. Physical copies of these forms will be provided by your mobile IV nurse. If you wish to save time, forms may be filled out and sent to mobileiv@besowb.com prior to appointment.

Forms will be reviewed and cleared by your mobile IV RN and supervising NP. Although unlikely, they have the right to deem a client unsafe for infusion therapy. In this case, deposits can be refunded if appointment termination is deemed necessary by our NP and/or RN. To avoid therapy refusal, please fill out our Demographics and Health Information Form so it can be reviewed and cleared by our staff.

Disclosure:

All appointments and services are performed by our trained, professional registered nurses who are supervised and work directly under our board certified Family Nurse Practitioner. By scheduling an appointment and signing our forms/consent, you are agreeing that all health information to be true and fully disclosed to our health care staff. This information is vital for our team to appropriately assess compatibility and safety of injections given.

Age Requirement:

Our IVs and IM injections are only available to those 18 years or older.

HIPAA Privacy Policy:

Only authorized staff of Beso Wellness and Beauty have access to your public health information. This information will not be shared with or provided to anyone outside of our trained staff members. Do not enter any public health information on an unknown or non-secure website or source. Fill out our physical forms and hand directly to our staff to ensure information safety and confidentiality.
